I don't have all of them, but heres the ones I know:
Mario & Luigi : Comes out of a pipe and says "Here we go!"
Peach: Parasols onto the stage
Bowser: Comes out of fire
the Kongs: Both come of barrels
Wario: Rides his chopper onto the stage
Samus: Comes down on a save platform
Zamus: Her amour falls off
Sonic: Spin dashes onto the stage
Yoshi: Comes out of an egg
Link: Rides a tornado onto the stage. Its a reference to the original LoZ
Zelda: Dunno
Toon Link: Small smoke cloud. Kinda like when you killed an enemy in Wind Waker
Ganondorf: Dunno
Pokemon Trainer: Runs onto the stage and releases a Pokeball
Pikachu and Jigglypuff: Comes out of a Pokeball
Lucario: Dunno
Marth and Ike: Warp magic
Pit: Golden light. Kinda like Link's in 64.
Ice Climbers: Ride Condor onto stage
Meta Knight: Warps out of his cape
Kirby: Warpstar
Dedede: boxing arena carried by Waddle Dees
Fox & Falco: Arwing
Wolf: I'm not sure, but probably Wolfen
Capt. Falcon: Blue Falcon
Ness: PK Teleport
Lucas: Mr. Saturn Table
Olimar: I'm not sure, although there are videos with him that I could find.

Also, lucario SHOULD come out of a pokeball
he's not l33t or badass enough to just warp in or something, if jiggs and picachu come out of a pokeball, so should lucario.

See this annoys me. Lucario is a pokemon as well, Picachu doesn't jump on the stage volt tackle stylee. Why should lucario look all mysterious and cool?Lucario "Teliports" onto the stage.
Pokemon should come from pokeballs!

That's kinda weird that the other pokes come out of balls and Lucario teleports in... I guess he's just THAT cool
