I think it's worthy to note that you are a little off in your assumption. Meta Knight did not hit the opponent behind him in that instance.

When you miss anyone entirely with the cape, nothing happens. However, when you nail it, as you know whoever was in the cape gets hit by a very powerful attack and takes a large amount of damage. Additionally, everyone on the screen gets hit by a weaker attack. That's what happened there.

It hits above him, too. About the height of aBattlefield platform, I saw it in a MK vs Sonic vid.

Mk hits the opponent behind him because the slash (or Special Smash Hit) start with a movement like a U...start in his back to have more power, and later he use that range to connect the Slash to the opponent in his front. That start is the hit that you see in the video when the third Jiggly was struck.
