This is done by too much momentum going in the opposite direction or too much of a slant when you are grabbed.

This usually happens when I shield grab someone while Im at the ledge... the momentum from his attack hitting my shield carries you past the ledge (even though you got the grab), and since aerial grabs dont work, it breaks instantly.
